Minimally-Invasive
Endodontics

Track #4

Day One of a Two-Day Course

Course Description

Prosthodontists and restorative dentists have finally convinced endodontists and general dentists that preservation of tooth structure during RCT is as critical to the longevity of endodontically treated teeth as successful treatment of root canal systems is to salvaging diseased teeth in the short term. To this end, endodonticspecialists have been testing the limits of minimally-invasive RCT and are encountering increasingly-difficult procedural challenges which invites the question, “How do we perform minimally-invasive endodontic procedures without losing our minds?” It is very clear we need new concepts, tools, and techniques designed specifically for MIE. Conceptually, we need to better understand where the law of diminishing returns tells us that further dentin conservation is misdirected, needlessly complex, and could actually lower success rates of RCT.

Learning Objectives

Day One:
■ Anatomy
■ Diagnosis and Treatment Planning
■ Profound Anesthesia for RCT
■ Designing and executing MIE access cavities
■ Instrumentation for minimally-invasive endodontics

Day Two AM:
■ Cleaning small preparations with closed-system negative pressure irrigation
■ MIE obturation techniques
■ Post-RCT restoration

Biography

Dr. Buchanan graduated first in his dental school class from The University of the Pacific in 1978. He received his specialty certificate from the Temple University Post-Graduate Endodontic Program in 1980. In 1986, he became a Diplomate of the American Board of Endodontics. Dr. Buchanan has practiced endodontics since 1980. In 1989, Dr. Buchanan was the first to reconstruct micro-CT scans of endodontic anatomy. He invented the Continuous Wave of Obturation technique in 1990, the first variably-tapered shaping technique in 1991, the first rotary negotiation technique in 2016, and the closed system negative pressure irrigation and filling technique in 2019. His instrument and device designs include: System-B; EOU; ElementsFree obturation heat sources; Buchanan Heat and Hand Pluggers; GT, GTX, and Traverse Rotary Negotiation Files; Buc ultrasonic tips; LAX guided access burs; TrueTooth and TrueJaw 3D-printed procedural training replicas; and most recently, the PulpSucker multi-canular negative pressure irrigation device. For over 30 years, Dr. Buchanan has shared his clinical competency by teaching procedural endodontics at worldwide events and at DEL, a teaching institute in Santa Barbara, California founded by Dr. Buchanan.

A Modified File Retrieval Technique Using a Newly Developed Lasso Device: The EndoCowboy

Track #4

Day Two of a Two-Day Course

Course Description

The frustration and difficulties related with the removal of separated instruments are well known. The presented file retrieval technique has the approach to be as minimally invasive as possible and more predictable in terms of time management, costs and outcome. A new troughing technique using sonic instead of ultrasonic reduces the time needed to expose the fractured instrument and minimizes the risk of a second fracture of the instrument as well as heat induced tissue damage. A specially developed wire with higher tensile strength and reduced elongation as well as a new ball bearing tensioning mechanism allows to remove even ingrained files using a strict push and pull movement. This allows a more minimally invasive file retrieval. The device and technique will be described in detail and a step by step tutorial will be presented during this lecture. Several cases will be shown to show the possibilities of this file retrieval technique.

Learning Objectives

Day Two PM: A minimally invasive and predictable file retrieval technique using the EndoCowboy

■ Creating the right access to the broken instrument
■ Troughing techniques with ultrasonic & sonic tips
■ Secure Lasso placement around exposed file segment
■ Precise and controlled operation of the EndoCowboy to unfold its full potential

Biography

Dennis Köhrer studied engineering at the RWTH Aachen University and received his dental degree from the private University Witten/Herdecke. He began to specialize in endodontics and microsurgery early in his career and is actively engaged in modern technologies of endodontic treatment. In numerous international courses and lectures he imparts his knowledge and has published several articles. Due to his engineering expertise, he is involved in the development process of dental products and has invented the Endo-Cowboy, an endodontic instrument for the retrieval of broken files. Together with his brother he is leading a clinic with seven doctors where he continuously strives to perfect techniques and instruments for a higher level of patient treatment.

WHAT MAKES LASCHAL® INSTRUMENTS SPECIAL…

LASCHAL® instruments are the best I’ve used in both design as well as in their awesome manufacturing. Their ouchless suture scissors allow me to remove sutures without hurting patients, and the way they designed the scissor joints so they never grab a suture is exceptional. The unique locking release is so much simpler than Castro Viejo’s suture needle pliers and it only takes a quick move of my thumb to release. Top that off with the remarkable range of their pliers and it is easy choosing LASCHAL®.

With LASCHAL® instruments, many endodontic procedures become much easier.
